Engaging Communities In Prevention
Healthy Heart Programming
The Need
Residents of the Twin Rivers and Lakes Region indicate that they have not regularly been screened for cholesterol and do not engage in regular moderate physical activity. Community assessment data indicates that the State wide average is significantly lower in five key risk factors: cholesterol tested within past five years, never having a cholesterol test, having a high cholesterol level, not engaging in moderate physical activity in the last month and the percent of people considered obese.
Actions
A collaborative group of healthcare professionals in the Twins Rivers and Lakes Region have joined together to participate in a two-year pilot study to build a community health model that is sustainable and can be replicated, and will address the need for heart health. The study targets populations at-risk for heart disease and focuses on prevention, as well as, treatment. Programming includes: heart health education via the media, free cholesterol screenings, community-wide physical activity programming and information and referral to community based health services.
Involvement
The Foundation for Healthy Communities is a not-for-profit corporation that exists to improve health and healthcare and includes New Hampshire hospitals, health plans, clinicians, home care agencies and public policy leaders. The Foundation and the network agencies of CCNTR are collaborating to facilitate this project.
