Coordinating Across the System of Care
The Community Care Tool-Box
The Need
Individuals and families face many challenges in receiving services in the CCNTR service area. Service agency staff also face gaps in services, turnover in staff, changes in service programming, eligibility changes, all of which impact the ability to provide coordinate referral and care to households in the region.
Actions
CCNTR is working with other service networks in the State to develop a shared set of software and electronic tools for service providers to use: Intake software will support - "any point of entry" for individuals to access services, eliminating duplicate paperwork and speeding servuce delivery to other providers. Eligibility screening and referral software will help professionals quickly review for and refer families to appropriate services. Care coordination software will create a common record for agencies to plan and deliver care to families and individuals with multiple needs. Agencies that maintain their own electronic records will have the benefit of data transfer to The Community Care Tool-box, with no additional keying. Individuals will maintain control of their care by granting permission for agencies to work together on their behalf. This "Community Care Tool-Box" will make it easier for individuals who receive or require assistance in the Twin Rivers Region to access care and services.
Involvement
Five NH service networks have joined together to become NHParTech Network. This coalition of coalitions has developed the vision and design of this progressive software program. Three of the "tools" in the toolbox are in development and early market testing. Others are in early development. This unique partnership arrangement has allowed service providers in NH to identify a gap in the service system, design and test a product that will meet the needs of NH rural networks and implement a community solution that will benefit consumers and providers of care throughout the State.
