    Bringing People Together

What started as a group of service providers getting together to discuss common problems in the Twin Rivers region, has become a strong and influential network of agencies, committees, and community activists, that have made some remarkable changes in the area.

The tri-county area known at the Twin Rivers includes fourteen rural townships in central New Hampshire:

    Alexandria Bristol Franklin Salisbury
    Andover Danbury New Hampton Sanbornton
    Bridgewater Hill Northfield Tilton
    Groton Hebron

Collectively, this geographic region is categorized as one of the poorest regions in the state, based on median income, number of persons below 185% of the poverty level, and other socio-economic factors. Residents of this area have traditionally been severely impacted by social problems related to poverty, including:

    1. Families whose basic needs (food, clothing, safe shelter, childcare, and transportation) are unmet.
    2. Difficulty in accessing health and social services.
    3. A need for health education and disease prevention.
    4. High-risk behavior among youth, including substance use and unplanned pregnancy.
    5. An under-funded system of public health services.
    6. Low citizen participation in community and civic activities.

By working together, the network has been able to develop plans to reduce the impact of these problems in our community. Early successes for the community involved Health First Family Care Center. This center was created by CCNTR in 1996 to provide health care services to uninsured individuals in the region. Low-income mothers are now able to receive prenatal care and take advantage of health education and prevention initiatives that have been proven to decrease the number of low birth-weight babies, and other related health risks. Another early success was the TRIP Center in Franklin, developed for intergenerational programs.

CCNTR established itself as a nonprofit agency in 1996. Since then, our network of dedicated agencies, individuals and funders have demonstrated how working together for healthier communities can and does make a difference to the people of the Twin Rivers area.
